Social Work services are available to all Nassau Health Care Corporation patients and their families
- A social worker can provide:
- Emotional support and counseling
- Address questions and concerns about benefits or
entitlements
- Assist with the discharge planning process - including
nursing home placement
- Provide information about community resources
If you'd like to speak with a social worker, you
can call the department at 572-4912 or ask your
doctor or nurse to contact Social Work on your behalf.

Patient Education
At the Medical Center, we believe that patient and
family education is a critical component of returning
to optimal health. Such education assists you and
your family members in understanding every aspect
of your care - your rights, condition, treatments,
rehabilitation, follow-up care and available community
resources. It is vital that patients become active
participants in their own care, armed with the skills
necessary to cope with a health care regimen at
home. All of your health care providers will be
happy to furnish you with the information you need
to stay well. HELP LINE: Our Department has established
a help line at (516) 572-4922 (Monday through Friday
from 8:30 am to 5:00 pm) to provide free confidential
counseling to recent trauma victims as well as their
families and friends. Callers will be helped to
understand common reactions to grief, the process
of mourning, emotional reactions to stress and trauma,
self-help strategies, when and where to seek further
professional help.
